A 60-year-old woman from China gradually lost her ability to communicate with her surroundings after suffering for about two months from deteriorating thinking and behavioral changes, until she stopped speaking completely. At the Qilu Hospital in Jinan, doctors tried to understand what was behind her condition, and after a series of tests, it turned out that her immune system had begun to mistakenly attack vital receptors in the brain. Following targeted treatment, significant improvement occurred, and after about half a year, she returned to full function.

Until the onset of symptoms, the woman was healthy, with no neurological disease or psychiatric background. However, within about two months, a significant change occurred in her condition. Her thinking ability deteriorated, her behavior became unusual, and communication with her became increasingly difficult. When she arrived at the hospital, she was no longer speaking at all.

Doctors also noticed involuntary movements that repeated again and again in the area of the mouth, jaw, and tongue. The combination of the loss of speech, the change in behavior, and the unusual movements raised suspicion that the problem was not purely psychiatric, but related to damage to brain activity.

An MRI scan showed unusual signs in several areas of her brain. The findings pointed to an inflammatory process, but did not immediately provide a clear answer regarding the cause. In fact, the picture was unusual even in relation to the disease that was discovered later.

The mystery was solved after doctors tested the fluid surrounding the brain and spinal cord. The test found antibodies against NMDA receptors — tiny structures on brain cells that play an important role in transmitting messages between cells and in actions such as learning and memory.

In simple terms, the body's defense system mistakenly identified part of the brain's communication system as an enemy and began to attack it. As a result, the activity of nerve cells was disrupted, and the woman began to lose basic abilities. The disease is called anti-NMDA receptor encephalitis.

Despite the severity of the condition, the disease has an important characteristic: the attack does not necessarily destroy the brain cells themselves. It impairs the way they communicate with each other, so when it is possible to stop the immune system's activity against the brain, there is a possibility that normal activity will return.

Doctors began giving the woman drugs designed to calm the immune system and stop the attack. Already after the first round of treatment, a significant improvement was recorded: her cognitive state improved and the involuntary movements began to disappear. Three months after the treatment, another MRI was performed, and it turned out that the unusual signs seen previously in her brain had almost disappeared. After half a year, she had already returned to full function and to the life she led before the disease began.

The disease diagnosed in the woman causes the immune system to mistakenly attack receptors important for brain activity. It can cause sharp changes in behavior, confusion, movement disorders, seizures, impairment of consciousness, and even loss of the ability to speak.

The case of the 60-year-old woman was unusual both because of her age and because of the findings seen in the brain scans. The disease appears mainly in young women, and in about half of the cases, an MRI of the brain may even appear normal. In this patient, however, extensive signs were found in several areas, which initially made it difficult to understand what was causing her condition.

The case highlights why it is important to identify the disease quickly: despite the severe symptoms, it is a condition that can be treated. In this case, the woman who arrived at the hospital after losing the ability to speak managed to return to full function within half a year.
