A person aged 50 or over may spend nearly an entire workday each year on various cancer screening tests. These procedures are often invasive and uncomfortable. While they save lives, we know they are just the tip of the iceberg, often detecting too few types of cancer and frequently at late stages. The world is craving a more convenient, comprehensive solution.

The Promise of Early Detection

Such a solution exists. The American company Grail, founded in 2015 by the genetic sequencing leader Illumina, has developed a blood test that promises to detect dozens of cancer types. In 2020, it launched a massive clinical trial with the UK's NHS. While initial results in February were viewed as a disappointment for missing the primary endpoint, full data published in May painted a more nuanced picture.

Sir Harpal Kumar, Chief Medical Officer and President of Grail, who previously led Cancer Research UK, remains convinced of the test's value. "We realized that early detection is the key to quality cancer treatment," he says. "I hadn't seen a company before Grail that offers one simple test for so many types of cancer."


Navigating Market Skepticism

The trial involved 143,000 participants aged 50–77. While Grail did not meet its primary goal, it successfully shifted cancer detection from stage 4 to stage 3, with a 16% increase in early-stage (1 and 2) detections.

Kumar emphasizes the clinical impact: "In colon cancer, five-year survival at stage 4 is 11%, while at stage 3 it is 64%. This is very significant." Experts suggest that as computing power and signal processing improve, the test's ability to separate cancer signals from noise will only grow.

The Future of Medicine

Grail’s core technology uses advanced genetic sequencing to examine genome methylation—an epigenetic process. "Methyl changes testify to almost everything we have gone through," Kumar explains. "Our unique knowledge is distinguishing cancer-related changes from those that are not, using deep machine learning on vast datasets."

Gradually, the company is learning to identify not just the presence of cancer, but its type, prognosis, and drug responsiveness. "We believe this is the future of treating all diseases," Kumar states. Although the test is currently awaiting FDA approval, it is already available in the US, Canada, and Israel. As competitors like Guardant and Exact Sciences advance their own trials, it is becoming clear that blood-based early detection is set to transition from a medical fantasy into a routine part of our daily lives.
